In some instances back pain may not be root pain or simple backache.

Examples include:

  • non-spinal pathology e.g. abdominal aortic aneurysm
  • systemic diseases where back pain is a presenting feature e.g. ankylosing spondylitis, tuberculosis (Pott's disease)
  • referred pain e.g. back pain radiating from the abdomen
