COBALT trial

The cobalt trial compared the efficacy of accelerated tPA with two bolus doses.

7169 patients with acute myocardial infarction were randomized to an infusion of 100 mg alteplase over 90 min or to two boluses of 50 mg separated by 30 min. Doses were reduced in smaller patients.

The mortality was higher in the double bolus group:

• double bolus 7.98%
• accelerated alteplase 7.53%
