The lesion which causes the Argyll Robertson pupil is usually sited in the periaqueductal gray matter of the midbrain.

The most common cause is neurosyphilis.

Other causes include:

  • diabetes mellitus
  • chronic alcoholism
  • encephalitis
  • multiple sclerosis
  • peroneal muscular atrophy
  • midbrain tumours
