  • only required for severe acute pulmonary histoplasmosis and acute disseminated histoplasmosis
  • treatment of choice is amphotericin
  • less toxic antifungal agents such as ketoconazole, itraconazole and fluconazole may be effective
  • surgical intervention may be required for the excision of histoplasmomas or chronic cavitatory lung lesions
