The presence of a light reflex with an intact accommodation reflex can be caused by:
- a midbrain lesion:
- the Argyll-Robertson pupil due to syphilis
- a ciliary ganglion lesion:
- Holmes-Adie pupil
- Parinaud's syndrome, usually due to:
- vascular events
- tumour
