vertical gaze palsy

A vertical gaze palsy is the loss of upward gaze.
This is discriminated from a third, fourth or sixth cranial nerve palsy because:
- both eyes are affected
- pupils are fixed but often unequal
- generally no diplopia
- generally the preservation of reflex eye movements, e.g. on flexion and extension of the neck
