    • chronic active hepatitis

  • associated with nephrocalcinosis:
    • primary hyperparathyroidism
    • vitamin D toxicity

  • tubulointerstitial disease:
    • obstructive nephropathy
    • chronic pyelonephritis
    • post renal transplantation
    • analgesic nephropathy

  • drugs:
    • amphoteracin
