The signs of retrocaecal appendicitis may include:

  • tenderness:
    • ill-defined anteriorly, but if localised it is usually lateral to McBurney's point
    • often most severe at lateral and posterior aspects of abdominal wall
  • spasm of quadratus lumborum on right side compared to left
  • positive tests:
    • psoas test
    • obturator test
