Initially, there is itching which is followed by a dry, scaly area which may become erythematous and purpuric. The skin is usually shiny and friable.

Brown hyperpigmentation is due to haemosiderin deposition derived from red cells extravasated from dilated, leaky capillaries.

The most common sites are the inner aspects of both lower legs above and around the medial malleoli.
