A closed mitral valvotomy is used in patients with mitral stenosis if the valve is uncalcified (loud first sound, opening snap). Classically the surgeon inserted a finger into the pulmonary valve. Today this technique has been superceded by balloon valvuloplasty.
Over enthusiastic valvotomy may result in mitral regurgitation. If required, this procedure may be repeated.
An open valvotomy is performed under direct vision with cardioplegia and heart byepass.
