Trigeminal neuroma is a rare intracranial tumour arising from the trigeminal ganglion or nerve root. It lies in the middle fossa or extends into the cerebellopontine angle, eroding the apex of the petrous bone.
Clinical features include facial pain, paraesthesia and numbness. Compression of posterior fossa structures produces nystagmus, ataxia and hemiparesis. These are usually of long standing.
Skull x-ray may reveal erosion of the petrous bone.
CT shows an enhancing lesion extending into the middle and/or the posterior fossa.
Surgical removal provides long-term benefit.
Trigeminal neuroma is one cause of trigeminal neuropathy.
