The most useful film is the erect chest radiograph, in which can be seen air under the diaphragm. This, however, has an important differential diagnosis as follows:
- perforation from the gut
- from outside, for example in trauma, surgery or peritoneal dialysis
- from the lung, through the diaphragm
- as a result of a gas infection
- through the vagina, for example after waterskiing
