Initial medical treatment should initiated before babies with severe cyanosis are transferred for surgical repair of the TGA.
The goal of medical intervention is to maintain the patency of the ductus arteriosus. The metabolic acidosis may need to be managed with alkali.
The overall goal is to effect a repair before right ventricular hypertropy and left ventricular atrophy occur. These are more likely after three weeks of age.
