Trans-sexualism is:

• the desire to be the other sex
• to be distinguished from transvestism
• mainly seen in men who wish to become female

If a man wishes to have a female phenotype then the surgery required is extensive, including procedures such as penile amputation, creation of an artificial vagina and breast implants.
