The histopathological features of mitral valve prolapse include:

  • an increase in the zona spongiosa of the valve leaflets

  • secondary changes:
    • formation of thrombus in the angle between the left atrial wall and prolapsing leaflet
    • endocardial fibrosis and thickening where the valve leaflet slaps against the myocardium
