THE BACK OF THE CERTIFICATE
The back of the certificate contains two boxes for completion where applicable:
- BOX A
- Put your initial here if you have formally referred the death to the
coroner. You do not need to do this if you have merely discussed with
the coroner whether to refer the death

- BOX B
- Put your initials here if you may later be in a position to give additional information about the cause of death (e.g. histological, microbiological, genetic, post mortem). A request for additional information will then be sent to the certifying doctor; for deaths in hospital, the request will be addressed to the consultant responsible for the care of the patient
