The differential diagnosis includes:

  • infection:
    • cellulitis
    • septic bursitis
    • aseptic arthritis

  • rheumatoid arthritis:
    • polyarticular gout that affects the fingers may be mistaken for rheumatoid arthritis
    • if there are elbow tophi these may be mistaken for rheumatoid nodules

  • bunion

  • chondrocalcinosis - pseudogout

  • seronegative arthritis
